Glucophage® XR & Me has been created by Merck Serono for Type 2 Diabetic patients who have been prescribed Glucophage® XR. It is intended to help manage aspects of your day to day routine relating to self-monitoring of blood glucose, medication adherence, prescribed treatment related appointments, diet and exercise.
With Glucophage® XR & Me you can:
- Log and track your blood sugar levels
- Set medication reminders
- Keep a food diary
- Record your walking distance through an inbuilt pedometer
- Keep track of your appointments
- Create and send progress reports
