The SHIFT prognostic model provides prognostic information in ambulatory patients with chronic systolic heart failure receiving contemporary guideline-recommended therapy. Ten parameters predictors for risk stratification in patients with chronic heart failure and left ventricular systolic dysfunction, including easily obtainable clinical characteristics have been included in this model.
